Senior Centers and Nutrition Programs
The AAA oversees operation of 18 senior centers and 2 Home Delivered Meal Nutrition Sites in the Southern Georgia region. Approximately 1,000 seniors receive meals daily either through the Home Delivered Meals program or through the Congregate Meals Program provided at senior centers.
AAA Programs & Services
Community Care Services Program (CCSP)
CCSP provides community-based and in-home services as an alternative to nursing home placement for frail persons who meet Medicaid eligibility requirements. A case manager acts as a service broker and client advocate to ensure that services help the client maintain independence and avoid nursing home placement.
Money Follows the Person (MFP)
Money Follows the Personis a joint effort between the Georgia Department of Community Health (DCH) and the Department of Human Services (DHS) that facilitates transitions through Medicaid waiver programs including the Community Care Services Program (CCSP), NOW/COMP, SOURCE and the Independent Care Waiver Program (ICWP). The goal is for states to develop alternative long-term care opportunities which will enable older adults and people with disabilities to fully participate in their communities.
The AAA's Wellness Program is designed to improve the health and quality of life of the elderly and to prevent or delay disability and premature death from chronic disease. The program assists individuals in making better lifestyle choices and in taking a more active role in their own well-being through exercise, nutrition, health education and medication management.
GeorgiaCares: Information and Assistance for Medicare Recipients
Linda Gail withGeorgiaCares provides counseling services to help Medicare consumers to (1) understand Medicare, Medicare Summary Notices, MediGap, Medicare Advantage Plans, Medicare Fraud and Long Term Care Insurance; (2) select a Medicare Part D prescription plan; (3) complete and file Medicare claims/appeals and prescription assistance requests; and (4) report Medicare fraud.
Long Term Care Ombudsman (LTCO)
Ombudsmen provide assistance to residents of nursing homes and personal care homes to resolve complaints, assure quality of care, maintain residents' rights, and provide education, information and advocacy on long-term care issues or concerns.
Elderly Legal Assistance Program
The ELAP program provides legal assistance, information, education and counseling services for older adults.
Alzheimer's Adult Day Care (ADC)
An adult day care program designed specifically for persons suffering from Alzheimer's disease and related dementias and their families is available in Lowndes County and the surrounding area.
Home Modification: Wheelchair Ramp Construction
This program is available to add safe and attractive wheelchair ramps to the homes of frail older individuals.
These services are available to individuals unable to perform one or more of the following activities: preparing meals, shopping for personal items, managing money, using the telephone or doing light housework.
Personal Care Services
This program provides personal assistance, stand-by assistance or supervision for individuals unable to perform one or more of the following activities: eating, dressing, bathing, toileting, transferring in/out of bed/chair, and walking.
Caregiver In-Home Respite
The respite program is available to provide a brief period of rest or relief for family caregivers of older adults. Are you a caregiver?
The Southern Georgia Regional Commission is contracted with the Department of Human Services (DHS) to coordinate transportation for older individuals. The AAA serves aging, TANF/substance abuse and mental health clients. Transportation is provided by- MIDS Transportation, Inc. currently under contract with the RC. The service provide approximately 130,000 trips annually.

Intake and Assistance
The I&A process may involve a telephone interview conducted by a Screening Specialist (a nurse or social worker) to determine an applicant's service needs. Upon determining the appropriate services needed, the applicant/client may be referred to a service provider for a face-to-face assessment.
