Gateway/ADRC staff members provide intake and assistance with accessing community resources and services.
Gateway/ADRC
staff also provides telephone
reassurance contact to frail, isolated, vulnerable seniors with no
family or friends to check on them.
What is an ADRC?
The ADRC initiative is part of a nationwide effort to restructure services and supports for older adults and persons with disabilities. It complements other long-term care system change activities designed to enhance access to community living. The goal of the ADRC program is to empower individuals to make informed choices and to streamline access to long-term support.
Programs & Services
The process of Intake and Assistance is the first step in being evaluated for programs and services. This step may involve a telephone interview conducted by a screening specialist (a nurse or social worker) to determine an applicant's service needs. Upon determining the appropriate services needed, the applicant/client may be referred to a service provider for a face-to-face assessment.
